REQ699911 –Level 3 Social Worker

This is an exciting opportunity for an experienced and highly motivated Social Worker to join the Social Work Department at St George Hospital. This permanent full-time position is based within the St George Hospital Transition Team and line managed by the Social Work Manager.

The Role

The Transition Team Social Worker plays a key clinical leadership role in managing some of the hospital’s most complex patient journeys. The team works to ensure effective care coordination and seamless transitions across all phases of care, while providing specialist support and consultation to clinical teams to address barriers to discharge, reduce avoidable delays, and minimise preventable readmissions.

The position carries an independent clinical caseload and provides expert consultation to Social Work colleagues managing patients with significant psychosocial, legal, and system complexities. The role requires advanced clinical reasoning, advocacy, and collaboration with internal and external stakeholders to achieve optimal patient outcomes.

The Transition Team consists of a Clinical Nurse Consultant, Occupational Therapist, and Social Worker, with specific portfolios allocated across the team. Currently, the Social Work portfolio focuses primarily on patients requiring complex discharge planning, including residential aged care pathways, NCAT applications, and NDIS access and planning processes.

The ideal candidate will demonstrate experience and knowledge in:

  • Complex case management and comprehensive psychosocial assessment.
  • Developing and maintaining collaborative relationships with residential aged care providers and managing complex aged care placement pathways.
  • Preparing high-quality briefing notes and reports for Hospital Executive and the Ministry of Health.
  • Understanding immigration-related matters that may impact healthcare access and discharge planning.
  • Knowledge of NCAT processes and their application within the healthcare setting.
  • Knowledge of NDIS processes and their application within the healthcare setting.
  • Experience with the provision of clinical supervision to level 1- 2 staff.
  • Has worked as a qualified social worker for at least 5 years.

You will join a large, dedicated, and highly skilled Social Work Department that provides services to a culturally and socially diverse patient population and their families. The successful applicant will also be required to support service delivery across other areas of the hospital as needed and participate in the after-hours on-call crisis roster.

Applicants should be aware that the position involves working with vulnerable individuals and people who may have experienced significant trauma and adversity. This role requires a high level of resilience, professionalism, and commitment to patient-centred care.
