Make a difference in the lives of individuals and whānau across our communities.

The Fono is looking for an experienced and compassionate Outreach Social Worker to join our team. This is an opportunity to make a meaningful difference by providing community-based social work support, advocating for whānau, and helping people navigate challenges to improve their overall wellbeing.

Working alongside a collaborative, multidisciplinary team, you will support individuals and whānau through a strengths-based, culturally responsive and whānau-centred approach.

About the Role

As our Outreach Social Worker, you will work directly with individuals and whānau in the community, providing professional social work interventions, assessment, case management, advocacy and service coordination.

You will build trusted relationships, identify barriers to wellbeing and connect people with the support and resources they need to achieve their goals.

Your work will include:

  • Assessment and case management: Undertaking comprehensive assessments, developing individual and whānau support plans, managing a caseload and monitoring progress.
  • Social work intervention: Providing holistic support to individuals and whānau experiencing social, emotional, financial, housing, health and wellbeing challenges, including crisis intervention where required.
  • Advocacy and empowerment: Supporting whānau to navigate complex systems, access services, understand their options and build independence and resilience.
  • Service coordination: Facilitating referrals and working collaboratively with health providers, government agencies and community organisations to ensure coordinated support.
  • Community engagement: Building strong community relationships, participating in outreach activities and identifying opportunities to improve access to support services.
  • Risk management and safeguarding: Identifying potential risks, responding appropriately to concerns and supporting safety planning to protect the wellbeing of individuals and whānau.

What You Will Bring

You are a confident and compassionate social work professional who can build meaningful relationships, navigate complex situations and work effectively with people from diverse backgrounds.

To be successful in this role, you will have:

  • A Bachelor of Social Work or equivalent recognised qualification.
  • Current registration with the Social Workers Registration Board (SWRB).
  • A current Annual Practising Certificate (APC).
  • A full, valid New Zealand driver’s licence.
  • Experience working with individuals, families and vulnerable populations.
  • Demonstrated experience in social work assessment, case management and developing support plans.
  • Strong communication, relationship-building, advocacy and problem-solving skills.
  • The ability to manage a varied caseload, competing priorities and accurate case documentation.
  • A commitment to professional ethics, confidentiality, safeguarding and ongoing professional development.

It would be advantageous if you also have:

  • Experience in community-based social work.
  • Knowledge of New Zealand’s health and social service systems and community resources.
  • An understanding of Te Tiriti o Waitangi and its application in practice.
  • Experience working with Māori, Pacific Peoples and other diverse communities.
  • Pacific language capability and/or Te Reo Māori.
  • An understanding of Pacific values and Māori and Pacific frameworks of wellbeing.
